The successful candidate will be expected to co‑coordinate the progress of patients through the Greater Manchester MSCC Service by ensuring a proactive, comprehensive, streamlined process which revolves around the patient and their individual needs. They will ensure a smooth process and compliance to national and local waiting time targets and other national, regional and local standards. Providing advice and education services relating to MSCC to The Christie and other partner Trusts across the regional network. Working in a team‑based clinical support service, providing cross‑cover and continuity of the service. The successful candidate will also act as a knowledgeable expert, who understands the process and stages of the patient care pathway specific to MSCC. The post holder will provide and receive complex, sensitive information working in partnership with other Greater Manchester health and social care teams. You will be expected to develop effective working relationships and actively contribute to patient flow enabling capacity within both outpatient and inpatient services. The post holder will display highly developed specialist knowledge of MSCC, understanding the roles of acute oncology in the management of both surgical and non‑surgical MSCC patients, as well as having a thorough understanding of the relevant legislative frameworks for clinical practice. The post holder will be expected to plan and organise their own workload and the ongoing work programme of the MSCC coordination team in conjunction with the wider team in a flexible manner. They will also be expected to provide advanced level of clinical coordination and offer both patients and colleagues highly specialised advice concerning MSCC pathways.
DUTIES AND RESPONSIBILITIES
- Communication and relationship skills: The post holder will provide and receive complex, sensitive information. Examples of this include: Working in partnership with other health and social care teams within the Trust, at Salford Royal NHSFT and other partner Trusts in the region. Developing effective working relationships and actively contributing to patient flow enabling capacity within outpatient and inpatient services to meet all relevant demands. Working as the single point of contact for issues related to MSCC care and demonstrating a solution‑focused and resourceful approach for both professionals and when required the people who use our services. Supporting the Clinical Lead in representing the Trust in regional pathway. Chairing, supporting and participating in clinical, business and educational meetings to address areas such as routine operations, service improvement, incident and complaint management and resource utilisation. Prepare accurate and concise records and reports, both verbal and written.
- Knowledge, training and experience: The post holder will display highly developed specialist knowledge in the area of MSCC. Examples of this will include: Being a graduate‑level health care professional (such as a nurse, midwife, health visitor, allied health professional, or physician associate) with experience of co‑ordinating the flow of patients through a pathway.
- Analytical skills: The post holder will interpret complex signs and symptoms of varying clinical cases. Examples of this will include: Being able to discern from colleagues whether a patient should be accepted as a patient of the MSCC service or whether that colleague will be provided advice for further management of their patient.
- Planning and organisation skills: The post holder will plan and organise their own workload and the on‑going work programme of the MSCC coordination team, in conjunction with other members of the team, in a flexible manner. Examples of this will include: Leading local MSCC pathway meetings and ensuring that accurate records of agreed actions, with identified leads, are communicated quickly and the completion of actions can be demonstrated. Supporting the Clinical Lead and lead manager in ensuring the service continues to meet the needs of its patients and the Trust’s regional partners. Scheduling events, meetings, training sessions etc to meet the service needs. Organising audits etc to demonstrate clinical compliance with guidelines.
- Patient/client care: The post holder will provide an advanced level of clinical co‑ordination, and will offer patients and colleagues highly specialised advice concerning MSCC pathways. Examples of this will include: Supporting the flow of patients through the MSCC services. Identifying patients whose pathway is complex at the earliest possible stage. Providing support to all relevant clinical teams to support in the identification, progression, escalation and co‑ordination of pathways for MSCC patients. Directly supporting complex cases and ensure liaison with other services and teams to ensure that achieving responsive packages of care.
- Policy and service: The post holder will propose and develop service improvements for the MSCC service, and will support their implementation and assessment within The Christie and the wider regional network. Examples of this will include: Contributing to the development and implementation of local policies and procedures related to the MSCC processes. Working in partnership with colleagues, patients and carers to ensure that pathways are patient‑centred. Participating in senior and managerial meetings to share information, participate in problem solving and contributing knowledge and skills to improve outcomes for patients, carers and professionals.
- Finance and resources: The post holder will demonstrate an individual responsibility for the appropriate use of resources. Examples of this will include: Prioritising own workload and that of colleagues effectively and ensure that during periods of leave colleagues are provided with information to ensure timely and continuous care. Being a responsible user of equipment and devices as part of their own work. Maintaining stock control and requesting/ordering consumables and smaller capital replacements.
- Education and Training: The post holder will provide an MSCC education service as a core part of their role. Examples of this will include: Providing specialist input to the training of medical, nursing, AHP, administrative and clerical staff. This may be in a pre‑registration, post‑registration or in‑service setting. Preparing educational resources for use by themselves, by colleagues in the service or by other team members from across the network. Presenting at business, educational or scientific meetings, conferences or courses. Scheduling educational events. Assessing the knowledge, skills and experiential levels of colleagues and students.
- Information resources: The post holder will record and document information about the patients’ MSCC pathway. Examples of this will include: Promoting high standards of clinical data recording within the service. Ensuring accurate records are maintained. Providing information to clinical decision‑maker colleagues to support pathway management and escalation and communicating this to relevant colleagues.
- Research and development: The post holder will regularly contribute to innovative practice. Examples of this will include: Using data to ensure that the Trust is able to demonstrate compliance with national standards and to prepare research and audit publications and presentations. Supporting patients through the MSCC pathway when these patients are enrolled in clinical trials, be that trials focused on MSCC management or other trials. Assisting the Enhanced Practitioners in Clinical Trial Management in setting up trials relevant to the MSCC service.
- Freedom to act: The post holder will adhere to the Trust’s key policies and the standard operating policies of the MSCC service. Examples of this will include: Implementing the policies agreed by the regional boards/committees concerning MSCC. Providing clinical advice that is in line with the clinical policies agreed by the Clinical Lead.
- Physical effort: The post holder will be office based with the requirement to attend other areas of the hospital. Examples of this will include: Sitting at a desk and utilising a VDU for the majority of each shift. Moving from one area of the service to another, potentially whilst carrying a laptop or other equipment items of a similar weight.
- Mental effort: The post holder will be expected to concentrate on clinical cases for most of each shift. Examples of this will include: Assessing the clinical information specific to individual patients. Quality assuring the information provided on individual patients and ensuring data errors/omissions are rectified and flagged to the relevant persons.
- Emotional effort: The post holder will display emotional resilience. Examples of this will include: Supporting cancer patients, the vast majority of whom are in a terminal phase of their treatment trajectory. Supporting clinical colleagues in caring for patients who lack capacity around future care planning and cannot advocate for themselves.
