Responsibilities
- To uphold the values of The Limes Medical Centre.
- Provide excellent patient clinical and customer care.
- Provide a high standard of nursing care and a comprehensive screening and health promotion service.
- Develop, implement and audit chronic disease management according to the health needs of the practice population.
- Maintain accurate information systems, both computerized and manual.
- Provide continued clinical support as appropriate.
- Assess specific diseases in accordance with agreed medical/nursing protocols.
- Undertake diagnostic health screening, health surveillance and therapeutic interventions within a broader health promotion/public health strategy.
- Conduct comprehensive health assessments of patients at the point of registration, including chronic disease management, contraception, sexual health, cervical screening, travel health advice and immunisation following Patient Group Directives (PGD), well person/elderly health review, venepuncture and basic rule management.
- Ensure that all nursing practice is evidence-based, applying current research.
- Support and enable individuals, carers groups and communities to take action to meet their priorities for health.
- Select, carry out and interpret specific tasks and investigations.
- Identify nursing care needs of the practice population supported by the Lead Practice Nurse.
- Implement the Infection Control policy.
- Support students and be actively involved in training of staff at all levels.
- Ensure suitability and cleanliness of equipment used for patient care, and that stock levels are appropriate, leading in infection control.
- Initiate practice development.
- Maintain own Personal Development Plan (PDP) and attend appropriate training events.
- Attend and contribute to practice meetings and liaise with the multidisciplinary team.
- Be involved in the delivery of Salford Standards and QOF and Direct Enhanced services.
- Organise the annual Flu/COVID Strategy.
- Maintain accurate and up-to-date patient records.
- Support practice development plans.
- Understand and apply legal issues that support the identification of vulnerable and abused children and adults, and be aware of statutory child/vulnerable patients' health procedures and local guidance.
- Work in close collaboration with General Practitioners, other Primary Care Nurses, Allied Professionals, Social Services, voluntary sector, the ICB and other appropriate agencies to ensure continuity of patient care.
- Support the clinical governance agenda of The Limes Medical Centre.
- Work within the Code of Conduct for Registered Nurses and under the direction of the Clinical Lead.
- Potential exposure to distressed patients.
- Continually update and develop skills and knowledge, participating in training events provided by The Limes Medical Centre.
- Each member of staff has a responsibility for their own development and that of others.
