We are looking for an enthusiastic and motivated Clinical Support Worker to join our dynamic Community Mental Health Team. In this role you will provide high‑quality, compassionate care to older adults with dementia and functional mental health conditions, working collaboratively with a well‑established team to deliver personalised treatment plans and support patients and carers as they navigate complex mental health challenges. The role may also involve occasional work with working‑age adults to support the wider goals of the service.
Key responsibilities:
- Deliver care and support for service users within the community mental health setting under the guidance of a registered mental health practitioner.
- Provide a range of therapeutic interventions and support as outlined in the service user’s care plan.
- Report on the well‑being of service users to registered mental health practitioners.
- Liaise with agencies necessary to the health and wellbeing of the service user and their carers.
- Support patients and carers in navigating complex mental health challenges.
