We are seeking a motivated and proactive Band 6 Occupational Therapist to join our newly established and continually developing Complex Community Therapy Team. This dynamic and forward‐thinking service brings together Occupational Therapists and Physiotherapists to provide comprehensive assessments for patients with complex needs following a hospital discharge. This is an exciting opportunity to be part of a growing team shaping new pathways and models of care across Manchester. Your input will directly support safe, timely, and appropriate transitions from hospital to home, ensuring patients receive the right care at the right time.

A full driving licence and access to a car will be required.

Key responsibilities:
  • Carry a caseload of patients with complex clinical, cognitive and social needs.
  • Undertake comprehensive therapy assessments to inform decisions about long‐term care and support.
  • Work closely with hospital teams to identify suitable patients for community discharge, using strong clinical reasoning and confident decision making.
  • Complete referrals for equipment and additional services to facilitate safe discharge.
  • Conduct home-based Occupational Therapy interventions following discharge, adapting your approach to meet highly varied needs.
  • Apply knowledge of community services, coordinating multi-agency support to ensure seamless care.
  • Use effective communication, diplomacy and resilience when working with hospital staff, community teams, patients, and families.
  • Demonstrate understanding of the Mental Capacity Assessment process, contributing to or leading best interest meetings.
  • Utilise moving and handling skills and be confident recommending and providing appropriate assistive equipment.
