Hospital Discharge Team Bridging Care, Ensuring Recovery
We are seeking an experienced and enthusiastic Assistant Team Manager to help lead our West Locality Discharge Service and play a key part in delivering efficient, effective services for adults across Wiltshire. Our team is part of a supportive, county-wide discharge service. This role is based at Royal United Hospital Bath and covers other settings, including rehabilitation beds in care homes.
We offer a strong network of support, guidance, and professional development to support you in this role. As Assistant Team Manager, you will support the flow between acute and community settings, to achieve positive outcomes for customers. Central to this is embedding “HomeFirst” principles and ensuring timely, safe assessments and support delivery.
You will have a pivotal role in delivering a high-performing, customer-responsive service. You will supervise and support Social Workers and Social Care Practitioners, ensuring a culture of learning and development. We are looking for a confident decision-maker who understands the importance of performance and brings energy, enthusiasm, and a passion for doing things differently to achieve the best outcomes.
You will need strong, refined leadership skills to support, develop, and motivate a team of highly skilled professionals. You should hold a relevant professional qualification in social care or health - such as a degree or Diploma in Social Work or Occupational Therapy. A Level 5 management qualification (or equivalent experience) is essential, along with at least two years of management or supervisory experience.
