The advertised job is an exciting development to join the Cancer Nurse Specialist team in Urology. The successful candidate will work with the existing team to deliver nurse‑led telephone assessment clinics and manage patients through the Cancer Pathways including diagnostic imaging, biopsies, diagnosis and SMDT.

Responsibilities:
- Perform specialist personalised assessment and higher level discussion of patient’s holistic needs, resulting in personalised care planning and evaluation of care delivery using advanced clinical knowledge and clinical skills.
- Demonstrate higher level clinical care by managing a patient caseload providing expert assessment, planning and evaluation.
- Co‑ordinate and manage nurse‑led clinics within speciality and contribute towards a multi‑professional approach in the management of patients within the specialist area, especially in relation to ongoing care needs and discharge arrangements, including providing access to community cancer support.
- Establish and maintain effective communication with patients and carers/relatives.
- Support patients and their families and ensure that they receive the required information to enable them to self‑manage, lead or participate in their care delivery.
- Recognise changes in patient’s condition that require the intervention of others and case manage/refer on as appropriate.
- Assist and encourage the multi‑disciplinary team in resolving patient problems, by providing new and innovative models of case management.
- Lead the specialist nursing contribution to the service, delivering high standards of patient care and working in partnership with lead clinicians, Lead Cancer Nurse, Head of Operations, and Matron.
- Work with the Matron to promote the development of nurses.
