To support a female with Epilepsy and Complex Learning Disabilities. Our goal is to provide a space that enables them to live as independently as possible, while staying safe and connected to the heart of the local community. You will provide specialist care and support and deliver our vision to create ‘a world where people control their lives’ to the people AFG support in line with each individual’s personalised plan.
Key Responsibilities:
- Provide all health-related and social support to people with diagnoses such as Learning Disability, Autism and Mental Health.
- Support people who present with behaviours that challenge by using appropriate techniques and strategies.
- Provide safe medication and pain management to the people we support in line with their medication care plan.
- Manage medical equipment and supplies and monitor depletion, re-ordering as necessary.
- Attend all clinical mandatory and specialist training sessions to remain competent to deliver care.
- Report and record all incidents which cause or have the potential to cause harm in a timely manner.
- Support people to design, develop and own the support they receive.
- Enable people to make informed choices and increase their independence.
- Support people to exercise their cultural beliefs and practices.
- Promote and increase independence for the people we support.
- Ensure the people we support are not abused, neglected or ill-treated.
General Duties:
- Provide personalised support to people with complex care needs, including assistance with activities of daily living.
- Actively participate in helping the people we support to achieve their outcomes as per their agreed Personalised Plan.
- Support people to develop and maintain relationships.
- Support people to make informed choices around a healthy lifestyle.
- Connect people with their community in accordance with their wishes.
Training & Development:
- Participate fully in training and development in accordance with the Alternative Futures Group development programme.
- Attend and actively participate in supervision sessions and performance reviews.
- Develop and share skills to aid in the provision and improvement of support.
Person Specification:
- Evidence of embracing, demonstrating and applying AFG’s Values.
- A positive attitude to people with complex physical, learning disabilities and/or mental health problems.
- Ability to provide sympathetic, emotional and practical support to others.
- Good verbal communication skills and ability to listen sensitively to others.
- Written English skills, with an ability to contribute accurately to record keeping.
- Minimum of 2 years experience in a Health & Social Care paid or unpaid role.
- Willingness to attend all clinical mandatory and specialist training courses.
- Ability and willingness to work flexible hours, covering 24 hours/7 days.
