Key Responsibilities
- Clinical Responsibilities
- Conduct clinical medication reviews for patients with complex needs
- Manage prescribing for long-term conditions
- Identify and manage medication‑related risks
- Implement deprescribing where appropriate
- Provide patient education and support adherence
- Prescribing & Medicines Optimisation
- Ensure prescribing follows NICE guidelines and local formularies
- Monitor high‑risk drugs and undertake safety audits
- Support cost‑effective prescribing initiatives
- Manage repeat prescription processes
- Patient‑Facing Services
- Run clinics (face‑to‑face/telephone/online)
- Address medication queries
- Provide advice on minor ailments and medication usage
- Quality Improvement & Governance
- Contribute to audits and service improvement projects
- Support implementation of prescribing safety alerts
- Assist in achieving QOF and enhanced service targets
- Collaboration
- Work with GPs, nurses, practice staff, and PCN pharmacists
- Liaise with community pharmacies and hospital teams
- Participate in MDT meetings where required
- Training & Development
- Support and mentor practice staff on medicines‑related issues
- Maintain CPD and professional registration requirements
