We are a multidisciplinary team of Specialist Nursing, Physiotherapists, Occupational Therapists and Pharmacy working closely to assess and promote wellness and independence of the population of Chorley, Preston and South Ribble presenting with deteriorating frailty symptoms. Your role will be to support the Occupational Therapist and Physiotherapists within this team, undertaking assessments and carrying out treatment plans that fall within the scope of your skills and knowledge and under the direction of the therapists.
Responsibilities
- Assist Occupational Therapists and Physiotherapists in community settings, including home visits.
- Carry out assessments and treatment plans within scope, under therapists’ direction.
- Support patients with long‑term conditions that have complex rehabilitation needs.
- Assist in providing equipment and equipment assessment according to patient needs.
- Collaborate with a multidisciplinary team in community settings.
- Assist in the development of links with other community services.
- Contribute to service development, clinical audit and research, with a particular emphasis on high‑risk clients (e.g., falls, decompensating frailty, optimisation of occupation and independence).
Qualifications
- Experience supporting Occupational Therapy and Physiotherapy interventions in a community setting, including assessment and provision of equipment.
- Experience as a Technical Instructor/Assistant Practitioner preferred.
- Excellent communication skills, natural problem‑solving ability, and the capacity to work autonomously as well as part of a team.
- Ability to travel for home visits.
Working Arrangements
Flexible working patterns such as term‑time, part‑time, compressed hours and flexi‑time are considered.
