JOB PURPOSE
To provide a full general practice service across different settings according to the portfolio role of the GP and the needs of the service.
Main Responsibilities
- The post holder will be responsible for seeing patients in an identical manner to regular general practice work, taking responsibility for a patient's care and only referring back to their own GP where appropriate, consistent with ensuring every contact counts.
- The GP will provide telephone and video support to the community ACPs, nurses and therapists assessing patients in the community.
- The GP will support patients who are managed in a GP-run virtual ward.
- The GP will also support paramedics and the ambulance service for patients who do not need conveyance to hospital but may still need some medical intervention.
- Unpicking complex medical histories and support proactive care planning; those with an interest in frailty; and those who enjoy supporting the learning and development of advanced practice colleagues.
- To assist in development of the service, with incident reporting and suggestions for improvements through the appropriate system.
