This is a rewarding, hands-on role where you'll make a real difference every day. ABL Health has expanded the Healthy Lifestyle Service Gloucestershire with a new Tobacco Dependency Outreach Service. The service focuses on supporting people across Gloucestershire from priority communities to stop smoking, improve wellbeing, and make long term, sustainable changes.

As a Community Wellbeing Advisor (Smoking Cessation), you will work:

  • Directly with priority communities across Gloucestershire
  • Proactively engage people who may face barriers to accessing traditional health services
  • Provide tailored, one-to-one behavioural support to help people quit smoking
  • Recommend and support the use of appropriate nicotine replacement therapies (NRT)

You’ll work with one or across multiple specialist outreach pathways, supporting communities such as:

  • People affected by substance misuse
  • People experiencing homelessness or living in temporary or social housing
  • The LGBTQIA community
  • The Polish community
  • Other underserved groups

How You’ll Make a Difference

You’ll be at the heart of a community-based, outreach-focused service by:

  • Delivering evidence-based stop smoking support through one-to-one, group and peer support sessions
  • Using motivational interviewing, health coaching and behaviour change techniques to empower clients
  • Creating personalised quit plans tailored to individual needs, preferences and circumstances
  • Actively promoting the service at community events and outreach sessions
  • Building strong relationships with local partners such as housing providers, substance misuse services, community organisations and the local authority
  • Helping shape and improve the service through insight, data and feedback from the communities you work with

All practice is grounded in the National Centre for Smoking Cessation and Training (NCSCT) standards. Full training and ongoing professional development will be provided.

Key Duties and Responsibilities

  • Work with key priority groups such as substance misuse, homelessness, living in temporary accommodation or permanent social housing, the Polish community, the LGBTQIA or other underserved communities.
  • Motivate, empower, and support clients from priority communities to make positive behavioural changes to address their smoking addiction and reduce health inequalities.
  • Deliver evidence-based stop smoking interventions (one-to-ones in community settings, community groups, telephone and online support), behaviour change and ongoing motivational support in line with the NCSCT Standard Treatment Programme.
  • Have effective communication and people skills to support people from priority groups to quit smoking.
  • Be competent in delivering personalised care, ensuring interventions are tailored to the needs and preferences of the individual.
  • Maintain current knowledge of evidence-based practice and integrate new research into service delivery.
  • Engage and recruit potential clients at promotional and community events.
  • Manage an active client caseload ensuring continuity of care by providing administrative support for appointment scheduling, confirmations, cancellations and follow-ups.
  • Lead peer support groups to increase people's confidence and motivation to quit smoking and to prevent relapse for successful quitters.
  • Work collaboratively with other professionals to embed smoking cessation within routine client care.
  • Attend regular Stop Smoking Service meetings and relevant training (internal and external).
  • Identify opportunities for service development and continuous improvement.
  • Provide high-quality, efficient, and cost-effective support across diverse community settings.
  • Support and contribute to ABL Health corporate initiatives as needed.
  • Work flexibly within the organisation to undertake such other reasonable duties and responsibilities, at any location within reasonable daily travel from your main place of work including weekends and evenings when required.

  • Ensure effective risk management and safeguarding practices, escalating concerns appropriately.
  • Support the Project Service Lead in performance management through reporting and service monitoring.
  • Record, monitor and maintain accurate, up-to-date client and service records using the ABL Health data system in line with ABL Health documentation policies.
  • Ensure all records are accurate and compliant with organisational policies.
  • Attend required meetings, maintain accurate documentation, and meet key deadlines.
  • Contribute effectively to team meetings, supervision, and service planning.
