Location: General Practice (Wales)

Hours: 2 full days per week (Monday and Thursday)

Pay: Dependant on experience estimated Band 6 equivalent (non-Agenda for Change practice pay structure)

Job Summary

The Practice Nurse will provide high-quality, evidence-based nursing care within a general practice setting, working autonomously as a registered practitioner within their scope of practice. The post-holder will play a key role in improving the quality, consistency, and coverage of targeted patient reviews, with a primary focus on Chronic Kidney Disease (CKD) and Cardiovascular Disease (CVD) quality improvement (QI) activity. The role will also support broader chronic disease management, preventative care, and health promotion.

Key Responsibilities

  • Quality Improvement & Targeted Patient Reviews (Core Focus)
    • Lead and support CKD and CVD quality improvement activity, including:
      • Identification, recall, and structured review of eligible patient cohorts
      • Delivery of comprehensive clinical reviews and optimisation of care pathways
      • Monitoring and interpretation of key clinical indicators (e.g., BP, eGFR, lipid profiles, albuminuria where appropriate)
      • Cardiovascular risk assessment and prevention optimisation
      • Improve and maintain clinical coding accuracy and data quality
      • Recall systems and patient engagement processes
      • Achievement of QI and contractual targets
      • Contribute to audit cycles and evaluation of CKD/CVD service outcomes
  • Chronic Disease Management
    • Undertake structured reviews for long-term conditions, including:
      • Diabetes
      • Hypertension
      • Asthma and COPD
      • Cardiovascular disease
    • Provide evidence-based education, lifestyle advice, and self-management support.
    • Ensure care is aligned with national and local clinical guidance.
  • Treatment Room & General Nursing Care
    • Provide core treatment room services including:
      • Wound care and dressings
      • Suture and clip removal
      • Injections (where trained and competent)
      • Opportunistic care and minor procedures within scope of practice
    • Ensure safe, timely, and evidence-based clinical interventions.
  • Prevention, Screening & Immunisations
    • Deliver vaccination programmes in line with UK and Welsh schedules
    • Cervical screening (if appropriately trained and accredited)
    • Health promotion and preventative care interventions across the patient population.
  • Clinical Governance & Professional Practice
    • Work in accordance with the Nursing and Midwifery Council Code of Professional Conduct
    • Maintain accurate, timely electronic patient records
    • Participate in audit, significant event analysis, and quality improvement activity
    • Adhere to infection prevention, safeguarding, and medicines management policies
    • Maintain professional registration and ongoing CPD in line with revalidation requirements
  • Communication & Team Working
    • Work collaboratively with GPs, practice pharmacists, and administrative staff
    • Support multidisciplinary care planning and delivery
    • Communicate effectively with patients regarding diagnosis, management, and preventative care
  • Service Development & Future Adaptability
    • Contribute to evolving primary care priorities within NHS Wales contractual and QI frameworks
    • Support ongoing service redesign and improvement initiatives
    • Adapt to changing quality improvement and population health requirements over time

Person Specification

  • Essential:
    • Registered Nurse (Adult) with valid NMC registration
    • Experience in primary care or community nursing
    • Competence in or willingness to develop chronic disease management skills
    • Ability to work autonomously across structured clinical sessions
    • Strong organisational and prioritisation skills
    • Understanding of safeguarding principles (adults and children)
    • Commitment to evidence-based practice and quality improvement
  • Desirable:
    • Experience in CKD and/or CVD management
    • Cervical cytology qualification (NHSCSP accredited)
    • Immunisation and vaccination training
    • Experience working within NHS Wales primary care systems
    • EMIS Web or equivalent clinical system experience
    • Experience or interest in quality improvement methodologies (QI cycles, audit work)
