An exciting opportunity has arisen for a Band 4 Tissue Viability Assistant Practitioner to join our specialist Tissue Viability Nursing (TVN) service within Central Cheshire Integrated Care Partnership. This is a key clinical role supporting the delivery of high‑quality, evidence‑based wound care across community and acute settings. You will work closely with the Tissue Viability Specialist Nurses and wider multidisciplinary teams to support patients to achieve optimal wound healing, prevention of harm and improved quality of life. This post offers an excellent opportunity to develop skills within tissue viability, working closely with patients, their families and carers, alongside opportunities to contribute to education, training and service improvement.
Responsibilities
- Support the Tissue Viability Specialist Nurses in the assessment, planning, delivery and evaluation of person‑centred wound care.
- Deliver evidence‑based tissue viability interventions in line with Trust policies, clinical guidelines and best practice.
- Maintain accurate, timely and professional clinical documentation using systems such as EMIS and other relevant digital platforms.
- Support and deliver formal and informal education and training to staff within the community and hospital settings.
- Act as a role model for best practice in tissue viability, infection prevention and patient‑centred care.
- Contribute to service development, audit and quality improvement initiatives.
- Work collaboratively with colleagues across community nursing, acute services and the wider multidisciplinary team.
- Promote patient dignity, safety, safeguarding and professional standards at all times.
Qualifications
- Holds a relevant Assistant Practitioner / Foundation Degree or equivalent qualification.
- Has experience delivering clinical care within community, ward or specialist services.
- Demonstrates an interest in tissue viability, wound care and skin integrity.
- Is confident communicating with patients, carers and healthcare professionals.
- Has good IT and documentation skills.
- Is committed to developing their skills, learning and contributing to service improvement.
- Works well independently and as part of a team.
- Shares the Trust’s values and commitment to high‑quality, safe, patient‑centred care.
This role is across community and inpatient settings and therefore applicants must hold a full UK Driving licence and have access to a car to use for work.
