Provide guidance and practical support to care leavers aged 16–25, helping them prepare for independence. Work in partnership with professionals across housing, health, education, and employment services. Co‑produce Pathway Plans with young people and key individuals in their lives. Maintain regular contact with care leavers and ensure access to the Cheshire East Care Leavers Local Offer. Deliver duty support services alongside other Personal Advisors and keep accurate, timely records.
You hold an NVQ Level 3 in Advice and Guidance or Health and Social Care. You have excellent organisational skills, strong personal resilience and a passion for improving outcomes for young people. You have experience working with diverse backgrounds and are committed to promoting positive outcomes for all care leavers. You enjoy building relationships and working in partnership with other agencies. You have strong interpersonal skills and thrive in a relationship‑based practice approach.
