An exciting opportunity has become available to join the Short-Term Services Team. There are two full‑time positions and one part‑time role (0.8 FTE, 29.5 hours per week). These vacancies are open to both progressed and non‑progressed Social Workers.

Responsibilities

  • Conduct adult needs assessments in line with the Care Act (2014).
  • Manage and respond to safeguarding concerns.
  • Apply the Mental Capacity Act (2005) to assess capacity and make best‑interest decisions.
  • Deliver person‑centred and evidence‑based practice.
  • Manage a diverse and complex caseload, including proportionate assessments and safeguarding.
  • Collaborate with multidisciplinary teams, hospital therapists, and wider Adult Social Care Teams.
  • Support patient flow through discharge pathways and assist with safeguarding risk identification.
