We are looking for an experienced Band 3 Healthcare Assistant (HCA) to cover up to 4 night shifts per week within the District Nursing Out Of Hours Service. Multiple roles are available and part‑time hours may be considered.

The role involves caring for patients who are approaching the end of life, working on a one‑to‑one basis and spending a full shift with a single patient in their own home. You will work without close supervision and must use your own initiative, following the patient’s care plan. You should be passionate about making a difference, helping patients to live as well as possible and to die with dignity. Strong time‑management, organisational skills and the ability to work independently are essential. Excellent communication and emotional strength are required. You must forge strong empathetic relationships with a variety of people while providing palliative care. Training and support will be provided on the job.

Responsibilities

  • Deliver nursing interventions to patients in their own homes across the CCICP footprint (Northwich, Winsford, Crewe, Nantwich and SMASH).
  • Monitor symptoms, undertake simple dressing and continence care, and keep detailed records.
  • Help patients meet their hygiene and dietary needs.
  • Work independently, adapt to other areas if required to meet service need.
  • Support peers, the team leader and the organisation in promoting the Trust’s values and behaviours.
  • Travel within the Central Cheshire Integrated Care Partnership footprint, with travel being an essential part of the role.
  • Maintain clear, concise and legible documentation, adhering to prescribed standards and protocols.
  • Use IT systems for relevant tasks such as accessing patient records and inputting data.
  • Obtain consent in accordance with Trust Consent Policy.
  • Report accidents/incidents in the clinical incident reporting system.
  • Maintain a patient‑orientated approach to care delivery, promoting a rehabilitative and self‑care approach.
  • Undertake phlebotomy duties as delegated by the registered nurse within the policies and procedures of the Trust.
  • Assist the Community Nursing Team in managing patients with long‑term conditions and end‑of‑life care.
  • Recognise the need for referral and liaise with other health‑care and multi‑agency workers as necessary.
  • Participate in induction and orientation programmes for staff requiring community experience.
  • Maintain knowledge of blood tests performed, select appropriate sample tubes and ensure correct labelling.
  • Maintain phlebotomy stocks, keeping them clean and in good order.
  • Follow Trust policy for needle‑stick incidents and report accurately on the Ulysses system.
  • Dispose of all syringes, needles and related materials in accordance with health and safety procedures.
