The predicted salary is between 46300 - 52800 Β£ per year.

The postholder will lead the day-to-day operational delivery of the Patient Flow and Discharge, deputising for the Deputy Director of Nursing and AHPs when appropriate. You will be responsible for ensuring our patients receive safe, efficient, high-quality, and cost-effective care across all their areas of responsibility, and work on Trust-wide initiatives to support timely discharge.

Responsibilities

  • Lead the operational delivery of services within the Directorate, ensuring performance is safe, effective, and patient-centred.
  • Support planning and delivery of exemplary services with robust governance and processes.
  • Hold line management responsibility for the services within the Care Group, Patient Flow and Discharge team, supporting personal development needs.
  • Ensure the Patient Flow Team provides 24/7 safe patient placement, appropriate management of inpatient bed allocation, professional and clinical leadership for all clinical areas, and leads the operational out-of-hours response to any critical or major incidents.
  • Ensure the Discharge Team delivers nursing leadership for safe patient discharge from the hospital, expert knowledge of complex discharge pathways, effective communication with partner organisations, and serves as a point of contact for staff, patients, relatives and other organisations.
  • Provide consistent clinical expertise, developing, implementing, and evaluating clinical elements of service redesign.
  • Coordinate first line response to major and critical incidents.

Qualifications

  • Registrant and clinical lead for the service.
  • Several years of experience at Senior Nursing, AHP, ODP, or HCS level with the ability to use clinical expertise when engaging stakeholders.
  • Ability to lead on the development, implementation, and evaluation of clinical elements of service redesign.
