Overview

Provide health assessment, support and care coordination for children and young people in care aged 0-25 years in St Helens.

Key Responsibilities

  • Maintain a caseload of Children in Care, producing comprehensive health assessments and developing individualized care plans.
  • Identify and manage health risks, collaborating with health and social care colleagues to provide inclusive primary health services.
  • Facilitate health education and health promotion to children, carers and partner agencies.
  • Deliver training and support to Children in Care teams and external stakeholders.
  • Ensure effective information sharing for children leaving care and capture the wishes, feelings and voice of each child.
  • Attend meetings and contribute to service improvement initiatives.
