We have an exhilarating opportunity for you to be a key player in our innovative, supportive Community Diabetes Service, dedicated to providing exceptional clinical care to adults in Bury, Heywood, Middleton, and Rochdale. As a vital link between primary and secondary care, our mission is to prevent unnecessary hospital admissions and enhance patients’ quality of life. You will be at the forefront of transforming diabetes management by offering essential support, expert advice, and comprehensive education to patients, their families, carers and fellow health professionals. Based at the modern Moorgate Primary Care Centre in Bury and the Phoenix Centre in Heywood, our community‑focused team delivers tailored diabetes interventions in various settings, ensuring that patients receive personalised care.

Responsibilities:

  • Act as liaison between primary and secondary care to prevent unnecessary hospital admissions.
  • Provide expert advice, education and support to patients, families, carers and other health professionals.
  • Initiate and adjust injectable and oral diabetes medications to empower patients in managing their health.

Qualifications:

  • Current NMC registration.
  • Up‑to‑date knowledge of the diabetes specialty.
  • Experience in managing complex diabetes conditions.
