The role supports the Team Manager in leading a dynamic group of social workers and practitioners who assist young people with care needs, disabilities, and complex transitions.

Responsibilities

  • Hold a reduced caseload of complex transition work.
  • Provide reflective support to enable the team to do their best work.
  • Drive excellent transitions planning aligned with the national Preparing for Adulthood framework.
  • Build effective partnerships across services, schools, colleges, and health.
  • Champion young people’s voices and ensure their aspirations underpin every decision.
  • Support the Team Manager with quality assurance, performance oversight, and developing best practice.
  • Model strengths‑based, person‑centred, and coproductive approaches.
  • Promote approaches that maximise independence, enabling young people to build the skills, confidence and autonomy needed for adulthood.

Standard working hours are 37 hours per week based on the usual business working week. Depending on service requirements, a rota may be required.
