Role: Adult Community Learning Disability Nurse within the learning disabilities service. The post holder will provide specialist nursing care to individuals referred to the learning disability team and work with service users and carers to promote independence and quality of life through assessment, care planning, implementation and evaluation of programmes of care and Health Action Plans.
Responsibilities
- Provide a high standard of specialist nursing care to people referred to the learning disability team and assist them to attain maximum independence and quality of life by assessing, developing, implementing and evaluating programmes of care and Health Action Plans in conjunction with service users and carers.
- Offer support to families and service users to gain maximum benefit from available services and resources.
- Take responsibility for the management of a caseload.
- Undertake a range of assessments in conjunction with service users, carers/families, using a person-centred approach; identify and support Health Facilitators for people with learning disabilities in accordance with the Valuing People: A New Strategy for Learning Disability for the 21st Century.
- Contribute to the Risk Management and Critical Incident Analysis reporting processes.
- Be involved in the setting, monitoring, maintaining and auditing of standards.
- Deliver a range of teaching sessions to individuals and carers of people with a learning disability.
- Support people with learning disabilities who may exhibit challenging behaviours, which may include verbal and/or physical aggression; devise strategies to deal sensitively with difficult behaviours in collaboration with carers, families and other professionals.
- Provide an efficient, timely and high-quality community nursing service to people with a learning disability and complex needs; develop and evaluate interventions and care plans.
- Take an active role in identifying and managing risk.
- Provide comprehensive specialist assessments to individuals with a learning disability.
- Work within a multi-disciplinary team to achieve the best outcomes for the individual with a learning disability.
Education and professional practice
- Work in accordance with Pennine Care Learning Disabilities policies and procedures.
- Collaborate with all members of the learning disabilities team to achieve Good Health objectives of Valuing People.
- Work with primary and secondary care services to ensure the health needs of people with learning disabilities are met.
- Attend study days and in-service courses as required; identify own training needs with assistance from the Line Manager; take personal responsibility for staying professionally up-to-date.
- Be involved in the education of the community about people with learning disabilities; participate in training programmes and educate service users, carers and others involved in their care, including Health Action Planning, Health Facilitation, Learning Disability Awareness and Challenging Behaviour.
- Maintain compliance with professional standards and, where applicable, professional membership requirements.
