Responsibilities
- Work as an autonomous nursing practitioner, named nurse for a designated caseload of children & young people (CYP) and their families.
- Collaborate with other professionals and agencies in the assessment, implementation, and evaluation of specialist care for children with long‑term, life‑limiting or palliative conditions.
- Provide and promote evidence‑based knowledge and skills to manage health outcomes for CYP and their families across community settings, ensuring safe care.
- Act in an advisory role to other professionals and agency staff regarding health care for CYP and families within your area of specialism.
- Support the Integrated Community Paediatric Service (ICPS) by participating in the service’s telephone 24‑hour on‑call and overnight home visits to support end‑of‑life care when required.
- Contribute to the further development of the ICPS, helping to ensure policy and practice are developed and delivered in line with Trust, regional and national best practice.
- Provide effective leadership across the service on issues within your area of specialism and scope of practice.
- Work in partnership with other health professionals and agencies to safeguard children and young people.
- Advocate, promote and deliver the highest standard of evidence‑based care for CYP and their families.
- Provide clinical knowledge to assess, identify, prioritise, implement and evaluate health assessments and programmes of care, and advise other professionals, CYP, families and non‑medical carers within your expert area of paediatric nursing.
- Promote continuous improvement for CYP and their families by collating and reviewing their experience.
- Communicate effectively with a wide range of people, including young people, families, health, education, social care colleagues and other relevant professionals.
- Provide line management responsibility for junior staff (Children’s Community nurses, Assistant Practitioners, Health Care Assistants).
- Promote a positive learning environment for all staff and visiting students, advising on professional development, ensuring supportive preceptorship and providing adequate mentorship for trainees.
