An excellent opportunity has arisen to recruit a Community Psychiatric Nurse/Care Co-ordinator into our busy Community Mental Health Team for Older Adults. The area covered by the post is Bolton Older Adults Community Mental Health Team. We are looking for an experienced, committed and motivated professional to work with older people who are living with dementia or functional illnesses. We are a friendly and supportive team and encourage professional and personal development.

Responsibilities

  • Complete Memory Assessments (training will be given)
  • Complete initial assessments
  • Participate in Duty Rota
  • Conduct Nursing Needs Assessments

We provide inpatient and community-based mental health care for people living in Bolton, Manchester, Salford, Trafford, Wigan, and across Greater Manchester and beyond.

  • Pay Enhancements – 30% additional for Evenings (8pm onwards) and Saturdays; 60% additional for Sundays and Bank holidays.
