Overview

Band 7 Clinical Practitioner – Planned Care (Long Term Condition Practitioner) within the Locality Integrated Neighbourhood Team at Birmingham Community Healthcare NHS Foundation Trust. We are looking for an experienced clinician with a strong background in the management of frailty and long‑term conditions to lead high‑quality, person‑centred care and contribute to service development and improvement.

Responsibilities

  • Lead and deliver holistic assessment, care planning and management of patients living with frailty and long‑term conditions across primary care networks and the wider locality.
  • Work collaboratively with multidisciplinary teams, including GP‑led Integrated Neighbourhood Teams, district nursing, therapy, mental health, voluntary sector and social care services.
  • Initiate and monitor treatment plans, ensuring continuity of care and supporting patients to manage their conditions and maintain independence at home.
  • Provide expert clinical input and decision‑making in complex care situations, promoting independence and reducing unnecessary reliance on services.
  • Coordinate and deliver services, collaborating with other health, social care and community services to promote seamless and person‑centred care.

Qualifications

  • Professional registration with the NMC or HCPC.
  • Qualified non‑medical prescriber and completion of a health assessment module.
  • Strong leadership and communication skills, with a commitment to person‑centred, evidence‑based practice.
  • Demonstrated experience in frailty and long‑term condition management.
  • Ability to work in partnership with GP and community networks to improve outcomes and quality of life.
