In a wide range of situations and locations to undertake the full range of activities required to deliver comprehensive mental health assessment, in an emergency for service users, who may be distressed and in crisis.
Key responsibilities include:
- The use of standardised assessment tools i.e. CAST, KGV, Recovery Star, History, strengths and aspirations, Mental state, Impact of culture and diversity, Functional needs, The needs of family and carer, Evaluation of risk, Physical health, Complicating factors, Identifying and ensuring specialist involvement where required.
- Developing, delivering and reviewing comprehensive and complex treatment programmes using appropriate frameworks in line with evidence-based practice, including strategies to manage risk for service users with complex needs and carers, bringing in other resources as required.
- Act as care coordinator for service users as appropriate, also providing defined interventions to individuals on other workers caseloads.
- Plan, deliver and evaluate defined, specialist therapeutic interventions as indicated in the assessment process, in line with personal recovery plans, including to service users who may be on other caseloads.
- Interventions may include: individual or group therapeutic intervention; psychological treatments such as CBT, DBT approaches, family interventions; psychosocial interventions; motivational and coping enhancement strategies; medication management; interventions under the Mental Health Act.
- Support a range of defined activities/interventions to improve carers' ability to support the service user and to enable them in their relationship with the service user.
- Develop and maintain partnership working with other services throughout all treatment episodes, including liaison with Primary Health Care Team, inpatient and intensive services, day services, voluntary sector and with nominated carers/advocates.
- Collaborate with service users and carers to facilitate development of crisis plans, rapid access plans, advance statements, involving other agencies as appropriate.
- Lead and develop recovery-focused relationships with service users, focusing on strengths and aspirations to enable meaningful personal recovery plans.
- Manage workload for self and planning for others, prioritising using electronic resources.
- Ensure safeguarding and protection of individuals in line with local policies and review protection plans.
- Coordinate protective interventions as required.
- Support service users to develop skills to manage their own health, using strengths-based, evidence-based approaches and personal recovery plans.
- Provide clinical leadership and supervision within the team; cover for manager and colleagues as required.
- Maintain health and social care records in accordance with standards; report within agreed timeframes.
- Participate in management, caseload and clinical supervision; provide mentoring/training for others in relevant practice areas.
- Contribute to learning objectives and assessments for students; develop own practice and contribute to development of others through feedback, supervision, coaching and appraisal.
- Adhere to professional codes of conduct and maintain required skills and competencies.
- Monitor health, safety and security of self and others; undertake assessments and act as required.
- Develop knowledge and practice and contribute to development of others; use feedback, supervision and appraisal to identify development needs.
- Lead, maintain and participate in practices enabling effective team working.
- Participate in local arrangements to manage unexpected staff absences.
