An exciting opportunity to join the Transfer of Care Integrated Team as a Transfer of Care Facilitator. We are a team based at the hospital that supports patients discharges from the acute wards and the community bed facility.

Responsibilities

  • Support wards and attend board rounds to ensure safe discharges.
  • Assess patients for Home First Discharges.
  • Provide patient information for the No Criteria to reside data.
  • Complete equipment assessments to ensure safe discharges.
  • Update electronic data systems daily with discharge plans.
  • Work Monday to Sunday 8:00‑16:00.
