An exciting opportunity to join the Transfer of Care Integrated Team as a Transfer of Care Facilitator. We are a team based at the hospital that supports patients discharges from the acute wards and the community bed facility.
Responsibilities
- Support wards and attend board rounds to ensure safe discharges.
- Assess patients for Home First Discharges.
- Provide patient information for the No Criteria to reside data.
- Complete equipment assessments to ensure safe discharges.
- Update electronic data systems daily with discharge plans.
- Work Monday to Sunday 8:00‑16:00.
