We are seeking an Occupational Therapist to join our multidisciplinary Community Rehabilitation Team on a fixed‑term basis. The post will suit an enthusiastic Occupational Therapist with an interest in developing skills across a wide range of therapeutic approaches. Our Community Rehabilitation Team supports individuals with complex and enduring mental health needs to achieve recovery and independence within the community across the Pennine Care footprint. The team is a trust‑wide service covering all five boroughs of Pennine Care and it is essential that you drive and have access to transport to travel across the whole footprint of the trust. We are currently based at Trust HQ in Ashton Under Lyne. Our core hours are Monday to Friday 9am‑5pm.
To embed the Occupational Therapy assessment and intervention pathway across the Community Rehabilitation Team, within the framework of Model of Human Occupation in collaboration with the wider MDT, you will be responsible for a caseload of service users under the care of the Community Rehabilitation Team, supporting them in their recovery journey. Responsibilities include:
- Undertaking specialist OT assessments in all areas of self‑care, productivity and leisure to support discharge planning and identify support needs.
- Conducting OT assessments, developing outcome‑based needs‑led care plans, facilitating the implementation of the agreed plan of care and evaluating its effectiveness utilising evidence‑based outcome measures.
- Providing a range of highly skilled interventions that will be delivered on both a group and individual basis and sharing those skills with other team members.
- Developing treatment plans and intervention to be delivered by the team’s social inclusion workers.
- Providing clinical supervision to the junior staff within the Community Rehabilitation Team.
- Leading the development of group‑based interventions to aid recovery and promote social inclusion.
- Facilitating the development of peer support networks as part of the discharge pathway from the service.
- Writing reports using MOHO outcome measures and ensuring up‑to‑date and accurate records are maintained regarding all patient contact.
- Managing a caseload of clients and coordinating safe and timely discharge with relevant professionals and families.
- Building partnership working with all relevant agencies/services/individuals to support optimal care for patients.
- Supervising the team’s technical instructor.
