Tameside's Adult Services is looking for a dedicated Assessor to join our Mental Health Team and do person centred reviews.

Key Responsibilities

  • Wellbeing and Needs Assessment: Engage with adults and carers to understand their needs and strengths, emphasizing early intervention and prevention.
  • Support Plan Development: Co‑produce support plans linked to strengths and needs, aiming for clear outcomes and integration with universal preventative services, assistive technology, or statutory services.
  • Mental Capacity and Best Interest Assessments: Conduct assessments where appropriate.
  • Safeguarding: Ensure safeguarding is a priority for adults with care and support needs at risk of abuse or neglect.
  • Risk Assessments: Review and complete risk assessments, promoting least restrictive practice.
  • Collaboration: Work with multiple professionals to support adults with complex needs, coordinating MDTs as needed.
  • Caseload Management: Maintain a manageable caseload and contribute to the duty rota, including occasional weekends.
  • Team Culture: Foster a team culture that values lived experiences and places them at the heart of the service.
  • Professional Development: Be accountable for your practice and ongoing learning, support colleagues and student placements.
  • Compliance: Adhere to departmental policies, procedures, and quality standards.
  • Decision Making: Exercise decision making with legal and ethical literacy.
  • Supervision and Reviews: Engage in regular supervision and Annual Development Reviews.
  • Service Improvement: Assist in continuous improvement programmes.
  • Record Keeping: Maintain accurate, person centred records using the case management system.
  • Efficiency: Collaborate with colleagues for efficient service delivery across all teams and areas.
  • Additional Duties: Perform other duties as required by the department or service.
