Responsibilities
- Lead and manage the nursing team in the Acute Frailty Hub.
- Coordinate patient assessments and oversee implementation of care plans.
- Ensure nursing practices align with best practice standards.
- Supervise junior staff, provide mentorship and training.
- Facilitate effective communication within the multidisciplinary team.
- Triage patients, manage clinical workflows, and ensure timely interventions.
- Lead quality improvement initiatives, patient safety protocols, and evaluate care outcomes.
- Advocate for service users and their families’ needs.
