Overview
An exciting opportunity to join the SMASH Care Community within the Central Cheshire Integrated Care Partnership (CCICP). The role involves assessing, managing and treating a complex caseload of patients with multiple therapy needs, providing supervision to junior staff and engaging closely with a multidisciplinary team that includes district nurses, advanced nurse practitioners and other specialists.
Responsibilities
Clinical Practice
- Manage a clinical caseload autonomously, undertaking comprehensive physiotherapy assessment and providing therapeutic diagnoses.
- Implement agreed treatment plans for complex service users with multiple pathologies respecting privacy, dignity and individuality.
- Deliver individual or group therapy as appropriate to the setting.
- Monitor and evaluate treatment outcomes to ensure effectiveness of interventions and service delivery.
- Develop programmes of care to support discharge from service.
- Ensure safe use of equipment, including competence and supervision of others.
- Record, maintain and store accurate, comprehensive and up‑to‑date documentation in line with legal and local requirements.
- Communicate outcomes of assessments/interventions to appropriate disciplines via letters, reports or discharge summaries.
- Arrange appointments, work closely with MDT, attend case reviews, conferences and other MDT meetings.
- Contribute to safeguarding of service users and families as appropriate.
- Assess capacity, gain valid informed consent and work within a legal framework for users who lack capacity.
- Maintain professional registration with HCPC and work within HCPC and CSP guidelines.
- Keep professional registration current, participate in CPD and adhere to national and local standards.
- Provide and receive sensitive information about service users from service users, carers and other professionals, managing sensitive or contentious information appropriately.
- Work in an empathic manner with service users, carers and families, overcoming barriers to understanding and communication.
- Communicate effectively with service users and carers to progress rehabilitation and treatment programmes.
- Use appropriate communication skills to overcome barriers to acceptance of sensitive information.
Leadership & Management
- Communicate effectively with a range of professionals both within the organisation and with external agencies.
- Provide information to Band 7 physiotherapists/managers to assist planning and service development.
- Support the use of open‑style leadership and contribute to strategic planning of the therapy service.
- Assist managers in achieving departmental objectives and support the daily management of workload allocation.
- Participate in operational planning, implementation of policy and service development, leading on delegated projects.
- Undertake additional duties as deemed appropriate by senior staff and deputise in their absence.
- Be aware of the financial impact of clinical decisions.
- Support the therapy team to ensure adequate levels of stock are available, following stock management policy.
Education
- Organise and lead in‑service training programmes for Band 5 therapists and assistants under the guidance of Band 7 staff.
- Critically evaluate current research and apply to practice.
- Demonstrate understanding and application of national guidelines and legislation relating to health and social care.
- Train, supervise and support less experienced therapy staff, assistants and students (formal staff appraisals may be included).
- Maintain competency to practice through participation in internal and external development opportunities, evidenced by a professional portfolio for CPD.
- Demonstrate acquired specialist skills and knowledge of professional practice to maintain fitness to practice as a physiotherapist.
- Adhere to HCPC Code of Conduct and all mandatory training requirements.
- Participate in the evaluation of clinical practice and disseminate findings across relevant areas.
Research, Audit & Governance
- Undertake measurement and evaluation of work and practices via evidence‑based projects, audit and outcome measures.
- Represent the service at local meetings, specific interest groups and attend national meetings under guidance of more experienced staff.
- Make recommendations for change and implement change under supervision.
- Undertake audit and contribute to regional/national level as appropriate.
- Support research activity within the service and disseminate findings to relevant areas.
Qualifications
Essential criteria
- Degree in physiotherapy.
- Registration (CSP) required.
- Previous experience of working in the NHS.
- Experience of working in a community setting.
- Good written and verbal communication skills.
- Good IT skills.
Desirable criteria
- Completion of Band 5 rotation.
- Student educator or experience of having students.
- Experience of audits.
- Experience of communicating with service users and carers.
- Extra qualifications relating to community physiotherapy.
Additional Information
You are required to be a car driver and have access to your own vehicle.
Occasional travel may be required.
