This is an exciting opportunity for a skilled nurse or health care professional to join the Admission Avoidance Team (AA). As a member of the AA Team, you will be expected to work as a senior clinician with a broad range of competencies who is able to adapt to a wide variety of clinical circumstances.

The Admission Avoidance team are based on the Acute hospital site and you will be expected to complete a holistic assessment for patients who are medically optimised across the Emergency Department, Acute Medical Unit, Integrated Assessment Unit, Same Day Emergency Care and Urgent Treatment Centre for discharge using a range of pathways including the home first pathway, discharge to assess pathway and Stamford unit referrals to avoid admission to hospital. This post will allow the successful applicant to gain an outstanding skill set and help to provide a seamless patient journey across acute and community areas.

You will also need to have knowledge of assessments and the Continuing Health Care (CHC) process and be able to complete fast-track and rapid discharges as well as supporting junior members of the team with complex discharges.

Main duties of the job:
  • To conduct holistic assessments within the hospital setting to support discharges and avoid admission by using pathways such as the home first pathway and discharge to assess pathway.
  • To work closely with other health and social care colleagues across the acute and community setting to support safe discharges and avoid admission to hospital.
  • Refer onto appropriate services to avoid readmission to hospital.
  • To attend unit board rounds daily, to identify patients who can be supported home with Admission Avoidance to facilitate earlier discharges.
  • Provide complex assessments and interventions and make high impact clinical decisions to support discharges and to support people to remain in their own homes.
  • You will be expected to work autonomously whilst drawing assistance from other colleagues in primary and secondary care.
  • You will use evidence-based principles, current best practice and specialist skills to assess, plan, implement and evaluate interventions.
  • Your caseload management and communication skills will enable you to prioritise patient care.
  • You will also work closely with senior clinicians, practitioners, social workers, and other health or social care professionals to establish and maintain a whole systems approach for patients in the service.
  • You will complete rapid discharges to support palliative and end of life patients to their preferred place of care and also complete assessments to support patients into an appropriate placement.

If successful this vacancy will require an enhanced DBS check at a cost of £54.90. This cost will be deducted from your first Trust salary. In addition to this it will be a condition of your employment that you will join the DBS update service and pay the annual subscription fee of £16.
