Responsibilities

  • Lead and manage the nursing team in the Acute Frailty Hub.
  • Coordinate patient assessments and oversee implementation of care plans.
  • Ensure nursing practices align with best practice standards.
  • Supervise junior staff, provide mentorship and training.
  • Facilitate effective communication within the multidisciplinary team.
  • Triage patients, manage clinical workflows, and ensure timely interventions.
  • Lead quality improvement initiatives, patient safety protocols, and evaluate care outcomes.
  • Advocate for service users and their families’ needs.
