We have an exciting opportunity to welcome Community Assessment Service Key Workers to our Llanelli and Ammanford team.
As a key worker, you will be working closely with people who have recently been discharged from hospital or deemed to require an urgent package of care. You will assess their needs and create a person-centred care plan for them, while also risk assessing their property and ensuring they each have the correct equipment in place. You will be liaising with external agencies, ensuring each service user on your case load is receiving the support they require. This role will be 80% service user contact time and 20% office based.
Minimum Requirements:
- Full driversβ licence and access to your own vehicle.
- Willing to obtain business insurance for your vehicle.
