Join a Growing Complex Care Provider

We are looking for an experienced and passionate Clinical Nurse to join our expanding Complex Care team. This is an exciting opportunity for a Registered Nurse who enjoys a varied role combining clinical assessments, care planning, service reviews, staff support and quality assurance within the community. Our office is based in Harrow-on-the-Hill, however the majority of our complex care packages are located across London, Reading, Slough and the wider South East region. The successful candidate will attend the office approximately one day per week, with the remainder of their time spent undertaking client assessments, supporting care packages and providing clinical oversight in the community.

Key Responsibilities

  • Conduct comprehensive clinical assessments within service users' homes.
  • Develop, implement and review person-centred care plans and risk assessments.
  • Support the mobilisation and ongoing management of complex care packages.
  • Provide clinical guidance and support to care teams.
  • Complete service reviews and quality assurance visits.
  • Undertake competency assessments and clinical observations.
  • Liaise with families, case managers, commissioners and multidisciplinary teams.
  • Ensure compliance with CQC standards, clinical governance requirements and company policies.
  • Support the growth and development of the Complex Care division.

Essential Requirements

  • Registered General Nurse (RGN) with active NMC registration.
  • Experience within Complex Care, Community Nursing, Homecare or Continuing Healthcare services.
  • Strong understanding of clinical governance and risk management.
  • Experience writing detailed care plans and conducting assessments.
  • Excellent communication and organisational skills.
  • Full UK Driving Licence and access to a vehicle.
  • Willingness to travel throughout London and surrounding regions.
