We're recruiting for a locum physiotherapist to work for a council in Central London. You'll be working as part of the council's discharge service and will be responsible for supporting residents in their return home from hospital.
This will involve assessing risk and designing interventions for various stages of the hospital pathway. For example, this could include the discharge to assess pathway or the post-discharge assessment of residents in their homes. Whatever the stage of the pathway, you'll be vital in supporting the residents' road to independence and preventing readmission to hospital!

- Support residents in their return home from hospital, promoting independence and helping to prevent readmission to hospital.
- Work as part of the council's discharge service in Central London.
- Contribute to the residents' road to independence through the discharge pathway.
This is a hybrid locum role, including both home and office working as well as working with residents in their homes. It offers a good degree of flexibility and independence when planning your week.
