About the role

Hours: 25-30 hours per week

The purpose of this role includes:

  • Provide up to 6 weeks of support to people recently discharged from hospital to help them remain independent in their homes and to access amenities in their communities.
  • Plan solutions with people that help them achieve their goals and contribute to their wellbeing.
  • Deliver person-centred support.
