CRT Care Services is seeking dedicated and compassionate Care Workers to join our Home From Hospital Service team. This service supports individuals in their transition from hospital to home, promoting independence through reablement and recovery ethos for all people accessing the service. You will be required to travel borough wide for this role.

Responsibilities:

  • Provide support to individuals returning home from hospital
  • Promote independence and recovery through personalised care plans
  • Assist with daily living activities and rehabilitation exercises
  • Monitor and report on the progress of individuals
  • Collaborate with healthcare professionals to ensure comprehensive care

Requirements:

  • Previous experience in a care worker role is essential
  • Must have a minimum of NVQ level 2 in Health and Social Care
  • Strong communication and interpersonal skills
  • Ability to work independently and as part of a team
  • Compassion and patient demeanour
  • Commitment to promoting independence and recovery

Pay and Shift Patterns:

Care Workers will be paid in block hours and shift patterns:

  • Monday to Friday: £14.00 per hour
  • Saturday to Sunday: £14.50 per hour

Location: Rotherham - Borough wide

Fleet cars available (must be over 21 to drive)
