Care Coordinator – Join Our Growing Team!

Location: Chesterfield (Derbyshire)

Salary: £27,963 with the potential to earn an additional £8000 per year from participation in the out of hours service

Hours: 37.5 hours per week based on Monday to Friday, 09:00–17:00 (with participation in out-of-hours on-call rota)

About the Role

As a Care Coordinator, you’ll play a vital role in ensuring the smooth delivery of care services. Your responsibilities will include:

  • Scheduling & Allocation: Matching the right carers to the right clients at the right time.
  • Communication: Building strong relationships with clients, carers, families, and healthcare professionals.
  • Record Management: Keeping all client and carer records accurate and up to date.
  • Problem-Solving: Staying calm under pressure and resolving issues quickly and effectively.

What We’re Looking For

  • Previous experience in health and social care (essential)
  • Excellent communication and organisational skills.
  • Ability to remain calm and patient under pressure.
  • A valid driving licence and access to a car (essential).
  • Flexibility to participate in the out-of-hours on-call rota (evenings and weekends).
