Clinical Nurse Specialist

This is a fabulous opportunity to be an integral part of the nursing services we offer our clients and to make a real difference to their lives and to the clinical care team.
You will work with Clinical Carers and complex care clients in their homes and support their access to the community. Ensuring our Clinical Team are trained and competent to deliver care. You will assess risks and needs to create individual, holistic care plans. The role involves telephone On-Call Duties.
* Current NMC registration to practice
* Experience working with children and adults in complex care or community settings
* Excellent communication skills and ability to train clinical staff
* Suitable for Nurse Band 5/6 with leadership and management experience
* Full UK Driving Licence required
Key responsibilities include:
- Building and maintaining relationships with clients and support networks
- Designing and delivering person-centred care plans
- Working with ICBs to establish care packages
- Supporting clinical team training
- Flexible working hours, hybrid working from home option
- Providing a laptop and mobile phone
- Access to Learning and Development, support with revalidation, professional registration fee reimbursement
- 25 days annual leave plus bank holidays
