Overview
Reablement at Home (R@H) delivers a regulated, seven‑day reablement service across North East Lincolnshire for adults aged 18 and over who are recovering from a hospital admission, experiencing a temporary decline in health or managing a disability. The service is person‑centred, aiming to restore confidence, develop daily living skills and maximise independence.
R@H is registered with the Care Quality Commission (CQC) and supports a diverse population, including older people, younger adults, people with disabilities and those living with dementia. All care plans are tailored to individual needs and promote health, wellbeing and dignity.

Responsibilities
- Lead and manage day‑to‑day operations, ensuring delivery of safe, effective and person‑centred care.
- Coordinate multidisciplinary teams and coordinate with external agencies such as hospital discharge teams, CUCT, voluntary organisations, GPs, therapists and care agencies.
- Maintain CQC registration requirements and ensure all regulated activities comply with legislation and best practice.
- Support the Operational Manager in achieving strategic objectives and driving continuous quality improvement.
- Apply the Mental Capacity Act, oversee capacity assessments and best interest decision‑making, and support staff in implementing evidence‑based practice.
- Collect, analyse and present service quality and performance data to inform decision‑making, service planning and improvement initiatives.
Experience
- Minimum of 2 years in a supervisory or management position.
- Experience in health and social care management and leading a team through change.
- Previous experience with the Care Quality Commission and managing CQC inspections.
- Experience in budget and financial management, service review and development including audit.
- Working knowledge of relevant legislation: Care Act 2014, Mental Capacity Act 2005, Equality Act 2010, Health and Safety at Work, etc.
- Experience in multi‑disciplinary teams or partnering with therapy, nursing and other disciplines.
- Experience in older people’s and learning disability services at a management level.
- Experience as a Registered Manager/Responsible Individual with the CQC.
- Proficiency in using databases.
Qualifications
- Professional qualification RN, AHP or Social Worker with current registration.
- Registration with a professional body.
- Safe Handling of Medication qualification.
