The Urgent Care Team is a 24/7 service providing a first point ofcontact and comprehensive triage service, telephone and clinical assessment topatients and/or their carers/relatives resulting in a safe and appropriateclinical decision and plan of care.
The Urgent Care Team is responsiblefor health and social care triage including supporting aversion of hospitaladmission or early hospital discharge through assessment and treatment ofindividuals in crisis, assessing complex and specialist needs in order todevelop, implement and evaluate programmes of care. Supporting individuals to remain in communitysettings, in their own home, residential home or nursing home for a shortperiod of time.

TheUrgent Care Assistant provides a short term crisis intervention service toindividuals within their own home environment that could include personal careand any support that has been identified through the assessment process.
You will work in partnership with nurses, physiotherapists, occupational therapists or care managers across the Intermediate Tier Service and undertake delegated duties after appropriate instruction in conjunction with a detailed support/outcome plan.
You will need to be able to work well asa team as well as using your own initiative and you will need to have goodcommunication and customer care skills.
It is essential that you have 2years experience of working in a health and social care setting and haveknowledge of the principles of enablement and self-care principles.

- Ability to maintain accurate and legible records

Experience
- Minimum of 2 years\β experience in working with adults in a similar setting
- Effective IT skills including use of M365 applications, including but not limited to Excel, PowerPoint, Outlook, Teams and Word
- Experience of working with the client group.

- Experience of Working in a Healthcare setting
Qualifications
- NVQ/QCF level 3 in Health and Social Care or completion on appointment
- Current driving licence
- Literacy and numeracy level 2 qualification
- Moving and Handling Certificate or willingness to undertake and complete
Other
- Weekends/Bank Holiday working will be part of an agreed rota pattern; however, staff are expected to be flexible to meet the needs of the service.
- Ability to transport self and equipment around the North East Lincolnshire area and within specified timescales
