Nurse Associate in Grimsby

The Community Nursing Service within North East Lincolnshire provides nursing care and support to patients, families and carers in a variety of community settings, supporting patients to remain in their own homes, preventing hospital admission and facilitating early hospital discharges, and managing end of life care. The Community Nursing Service works closely and in partnership with Primary Care Networks, Secondary Care, Social Care, Specialist Nurses and other healthcare staff to provide a quality service personalised to meet individual needs. The service supports and assists patients to manage ill health and long term conditions including elements of disability, thus allowing them to have the best possible quality of life, while maintaining dignity and respect.

Main duties of the job:

• You will work under the direction of the shift/team leader/manager in supporting other Health/Social Care Professionals and the delivery of Health and Social Care interventions to Service Users.
• You will be expected to independently manage and self-manage a defined caseload of less complex cases, evidencing problem solving and reasoning skills in response to unpredictable patterns of work.
• You will complete assessments and deliver support for the functional/clinical needs of patients in the community including chronic and palliative conditions.
• You will be responsible for developing and implementing care/support plans under the direction of the appropriate professional which may include clinical, social care and/or other practical activities with minimal guidance.
• You will monitor and report any unexpected changes in condition of the individual to the appropriate professional in a timely manner.
• You will manage care of individuals and carers where the support/care plan has been put in place and may need close monitoring, frequent review or where interventions to minimise and manage risk may be required.
• You will be required to utilise clinical skills including venepuncture and wound management with an ongoing responsibility to maintain and improve clinical skills and knowledge as service needs demand.

Person Specification:

• Qualifications: Registered Nursing Associate on the NMC register; Educated to a minimum level 5 Foundation degree level specific to nursing associate practice; Care Certificate or willing to achieve within 6 months of commencement in post.
• Knowledge & Skills: Ability to work autonomously in the community setting; Effective interpersonal and communication skills; Ability to organise and prioritise own workload; Able to recognise own limitations in the context of the nursing associate role; Know when to seek advice and refer to a registered health or care professional; Ability to participate in reflective practice and clinical supervision activities; Basic computer skills; Knowledge of Systmone computer system including mobile version; Able to demonstrate knowledge and skills to others; To have a range of clinical skills, such as, wound care, eye care, pressure area care, venepuncture, bowel/bladder care, clinical observations including blood pressure and blood glucose monitoring; Ability to maintain clear and accurate electronic and paper records; Knowledge and understanding of the regulations relating to Health and Social Care.
• Other: Flexible approach to working arrangements; Ability to transport self and equipment safely and in a timely manner; Ability to demonstrate commitment to CPG core values.
• Experience: Experience of working in a health and/or social care environment; Experience of performing clinical duties appropriate to the role; Experience of completing data entry using electronic systems; Experience of working in partnership conjunction with senior staff.
