The Community Urgent Care Team (CUCT) provides a comprehensive 24/7 health and social care triage service to patients and/or their carers/relatives with the aim of averting the need for hospital admission or early hospital discharge. This includes assessing complex and specialist needs in order to develop, implement and evaluate the appropriate programmes of care and wherever possible, supporting individuals to remain in a community setting whether it be their own home, a residential home or nursing home. The CUCT will continue to be involved with the individual's programme of care for up to 7 days if required.
Main duties of the job:
- Permanent – 37.5 hours per week
- You will be required to work a rota pattern of 12 hour shifts including weekdays, nights, weekends and bank holidays.
- Salary Range: £45,987 – £52,434 per annum
- You will be practising as an Advanced Clinical Practitioner (ACP) providing a first point of contact/triage for patients with undifferentiated and undiagnosed health problems.
- You will work predominantly in an autonomous and independent manner and will assess, examine, diagnose and implement a plan of care/treatment which may include follow up, discharge or referral to other health care professionals.
- This exciting and challenging role offers you the opportunity to provide high quality patient focused care in the community, providing clinical care.
- You will be responsible for health assessments including exacerbations, undifferentiated and undiagnosed of acute and chronic conditions.
- You will develop an understanding and application of social assessments in crisis.
- Community experience is desirable, however appropriate training and development will be offered to those with a wide range of transferable skills and experience.
