The Hospital Discharge Team are based at the Diana Princessof Wales Hospital. The HospitalDischarge Team is responsible for the safe and effective discharge of patientsfrom hospital into the community and also the prevention of admission tohospital.
The Hospital Discharge Team works in partnership with the Acute Trust ensuring effective prevention anddischarge pathways are followed. Thepostholder will provide professional support to those staff working in theHospital Discharge Team.
The team operates 7 days per week working between thehours of 8.00am and 8.00pm.
Main duties of the job

Hours: Part-time, permanent (up to 22.5 hours per week available) Less hours Will be considered
Youwill be expected to work as part of the HDT team under the direction ofthe Clinical Lead, to ensure the smooth flow of patients through the team. It is expected that by coordinating administrative tasks, and liaising with other departments, you will contribute to the efficient and effective running of the Discharge Team resulting ina significant reduction inpatient journeytimes and an effective discharge plan beingin place.
