As a Reablement Healthcare Assistant, you will travel from visit to visit across Oxfordshire (mainly Cherwell) to support people in their own homes. This is a fast-paced role in which you will follow care plans, complete assessments, and support clients in building confidence and independence.

SALARY: £28,600.00 per year (based on a 40 hour contract, pro rata for contracts between 30-40 hours)

HOURS: 30-40 hours per week, alternate weekends. Shift pattern: Working a mixture of early shifts (7am-3pm) and late shifts (2pm-10pm).

TRAVEL AND DRIVING: You must hold a full UK driving licence and have access to a car for work purposes. This is a driver's only role due to travelling between visits across Oxfordshire. You must also be able to commit to working alternate weekends. Fully paid travel time with 40p per mile mileage between visits.

DUTIES INCLUDE:

  • Having the confidence to complete assessments with new clients following discharge from the hospital
  • Carrying out reviews of clients' care plans as their needs change
  • Carrying out telephone monitoring calls to check on client progress and wellbeing
  • Supporting clients to regain independence with tasks, such as personal care, toileting, meal preparation and medication
  • Recording and reporting changes clearly and concisely, keeping the Home First team updated on improvements in abilities, allowing ongoing assessment of the suitability of the proposed care
  • Building strong working relationships with clients, colleagues and the Home First team
  • Being adaptive and responsive to changes in needs
