Support vulnerable adults in Oxfordshire following discharge from hospital. You will travel from visit to visit across Oxfordshire (mainly Cherwell) to help clients build confidence and independence in a fast‑paced role involving care plans, assessments and daily support.

Salary: £28,600.00 per year; 40 hours per week with alternate weekends.

Shift pattern: Early shifts (7 am‑3 pm), Late shifts (2 pm‑10 pm).

Travel and driving: Must hold a full UK driving licence and have access to a vehicle. Fully paid travel time with 40p per mile mileage. The role requires work on nights per week.

Right to work: Authorized to work in the UK. No sponsorship will be provided.

Duties:

  • Complete assessments with new clients after hospital discharge.
  • Support clients to regain independence with tasks such as personal care, toileting, meal preparation and medication.
  • Record and report changes clearly, keeping the Home First team updated on progress.
  • Build strong working relationships with clients, colleagues and the Home First team.
  • Adapt and respond to changing needs.
