What You’ll Do:
- As a key member of our interdisciplinary care team, you’ll support patients and families by providing emotional guidance, resource coordination, and advocacy in the hospice settings.
- You’ll collaborate closely with nurses, therapists, and physicians to ensure comprehensive care and a strong support system for every patient.
Your Day-to-Day Might Include:
- Offering emotional and psychosocial support to patients and families
- Helping patients and caregivers set healthy boundaries and goals
- Identifying high‑risk situations and coordinating timely interventions
- Referring families to community services and financial assistance programs
- Supporting hospice patients with bereavement and respite care planning
