What You’ll Do:

  • As a key member of our interdisciplinary care team, you’ll support patients and families by providing emotional guidance, resource coordination, and advocacy in the hospice settings.
  • You’ll collaborate closely with nurses, therapists, and physicians to ensure comprehensive care and a strong support system for every patient.

Your Day-to-Day Might Include:

  • Offering emotional and psychosocial support to patients and families
  • Helping patients and caregivers set healthy boundaries and goals
  • Identifying high‑risk situations and coordinating timely interventions
  • Referring families to community services and financial assistance programs
  • Supporting hospice patients with bereavement and respite care planning
  • Participating in team case conferences and care planning
  • Providing supervision to other social work staff (if licensed at LICSW level)
