At Care New England, we’re looking for dedicated professionals to join our VNA team and help patients and families navigate life’s most important healthcare moments right at home.
What You’ll Do
- As a key member of our interdisciplinary care team, you’ll support patients and families by providing emotional guidance, resource coordination, and advocacy in the hospice settings.
- You’ll collaborate closely with nurses, therapists, and physicians to ensure comprehensive care and a strong support system for every patient.
- Offering emotional and psychosocial support to patients and families.
- Helping patients and caregivers set healthy boundaries and goals.
- Identifying high-risk situations and coordinating timely interventions.
- Referring families to community services and financial assistance programs.
- Supporting hospice patients with bereavement and respite care planning.
- Participating in team case conferences and care planning.
- Providing supervision to other social work staff (if licensed at LICSW level).
- Documenting assessments, plans, and outcomes in a timely and accurate manner.

What You’ll Need
- Bachelor’s Degree in Social Work (BSW) required.
- Master’s Degree in Social Work (MSW) preferred.
