What You’ll Do:
- Manage a caseload of patients from admission through discharge, working as part of a dedicated interdisciplinary team.
- Assess, plan, and coordinate care while monitoring progress and adjusting treatment as needed to promote the best possible outcomes.
Your Day-to-Day Might Include:
- Performing comprehensive assessments and developing personalized care plans.
