At Care New England, we’re looking for dedicated PTs to join our Home Health team and help patients regain strength, mobility, and independence where they feel most comfortable.

What You’ll Do

  • Manage a caseload of patients from admission through discharge, providing comprehensive evaluations, developing individualized treatment plans, and delivering therapeutic interventions.
  • Collaborate with a supportive interdisciplinary team to achieve the best possible outcomes for each patient.
  • Perform in-home physical therapy evaluations and mobility assessments.
  • Develop and adjust personalized treatment plans based on patient goals.
  • Provide direct therapeutic interventions to improve function and independence.
  • Educate patients and caregivers on safe practices, exercises, and mobility tools.
  • Coordinate care with nurses, occupational and speech therapists, and case managers.
  • Document treatment and progress accurately and timely in compliance with policies.
  • Participate in interdisciplinary meetings and contribute to quality improvement.

What You’ll Need

  • Education and qualifications required for RI Physical Therapy licensure (required).
  • Bachelor’s Degree in Physical Therapy or related field (strongly preferred).
  • Valid Rhode Island PT license and CPR certification (required).
  • At least one year of clinical experience (home health or acute care preferred).
  • Valid driver’s license and automobile liability insurance.
