At Care New England, we’re looking for dedicated PTs to join our Home Health team and help patients regain strength, mobility, and independence where they feel most comfortable.
What You’ll Do
- Manage a caseload of patients from admission through discharge, providing comprehensive evaluations, developing individualized treatment plans, and delivering therapeutic interventions.
- Collaborate with a supportive interdisciplinary team to achieve the best possible outcomes for each patient.
- Perform in-home physical therapy evaluations and mobility assessments.
- Develop and adjust personalized treatment plans based on patient goals.
- Provide direct therapeutic interventions to improve function and independence.
- Educate patients and caregivers on safe practices, exercises, and mobility tools.
- Coordinate care with nurses, occupational and speech therapists, and case managers.
- Document treatment and progress accurately and timely in compliance with policies.
- Participate in interdisciplinary meetings and contribute to quality improvement.

What You’ll Need
- Education and qualifications required for RI Physical Therapy licensure (required).
- Bachelor’s Degree in Physical Therapy or related field (strongly preferred).
- Valid Rhode Island PT license and CPR certification (required).
- At least one year of clinical experience (home health or acute care preferred).
- Valid driver’s license and automobile liability insurance.
- Strong communication, clinical, and documentation skills.
